O vínculo terapêutico na clínica fonoaudiológica: uma reflexão necessária / The therapeutic link in the speech therapy clinic: a necessary reflection

RESUMO Objetivo Compreender os sentidos que o vínculo terapêutico assume para fonoaudiólogos clínicos. Método A pesquisa foi aprovada por Comitê de ética, sendo de caráter transversal, de abordagem quanti-qualitativa, pautada na Análise do Conteúdo. A pesquisa contou com a participação de 96 profissionais fonoaudiólogos clínicos, inscritos no Conselho de Fonoaudiologia da 3ª região (CRFa 3), o qual abrange os Estados do Paraná e de Santa Catarina. Resultados Dos 96 fonoaudiólogos incluídos, parte significativa dos participantes definiram o vínculo terapêutico como sendo relação/interação. Referente ao papel do vínculo para o trabalho clínico fonoaudiológico, a maioria dos profissionais o descreveu como base fundamental e outra parte deles afirmou que tal vínculo é necessário para a evolução/desenvolvimento do paciente. Conclusão É possível compreender que, de acordo com os participantes, a relação terapêutica é essencial para a sustentação e manutenção do trabalho clínico fonoaudiológico, impactando na ressignificação da queixa e na minimização do sofrimento dos usuários.

ABSTRACT Purpose To understand the meanings that the therapeutic bond assumes for clinical speech therapists. Methods The research was approved by the Ethics Committee, being of a transversal character, with a quantitative-qualitative approach in the Content Analysis. The research with the participation of 96 clinical speech therapists, registered in the Speech Therapy Council of the 3rd region (CRFa 3), which covers the States of Paraná and Santa Catarina. Results Of the 96 speech therapists included, a significant part of the participants defined the therapeutic bond as a relationship/interaction. Regarding the role of the bond for the speech therapy clinical work, most professionals declared theirs as a fundamental basis and another part of the bond is necessary for the evolution/development of the patient. Conclusion It is possible to understand that, according to the therapeutic patients, it is essential to sustain, maintain the clinical work for users, impacting the resignification of the complaint and the minimization of the users' suffering.

A terapêutica de um "CAPS AD" em um coletivo de fotografia / The Therapeutics of a "CAPS AD" in a Photography's Collective

Sendo fundamental a manutenção de um processo de reflexão constante para que a consolidação da Reforma Psiquiátrica possa se dar sem um "engessamento" de práticas que atrapalharia a proposta de atendimento personalizado, adequado à realidade específica de cada caso, faz-se importante observar as experiências que vêm acontecendo nos dispositivos de Atenção Psicossocial. Por isto, este artigo relata experiências ocorridas em um CAPS AD nos anos de 2011 a 2014, narrando o processo de implementação de uma oficina de fotografia e suas implicações. Percebeu-se que, propondo uma atividade aberta, flexível às demandas dos usuários, foi possível despertar interesses artísticos e de gestão, que culminaram no engajamento em um coletivo de fotografia - algo que não se deu por uma obrigatoriedade de frequência a atividades, mas por uma abertura da oficina aos interesses de cada participante, o que funcionou melhor para o aumento da "adesão".(AU)

Considering that it´s fundamental to maintain a process of constant reflection so that the consolidation of the Brazilian Psychiatric Reform can take place without a "plastering" of practices that would hinder the proposal of personalized care, adequate to the specific reality of each case, it is important to observe the experiences that have been happening in the devices of Psychosocial Attention. For this reason, this article reports the experiences that occurred in a CAPS AD in the years 2011 to 2014, narrating the process of implantation of a photography workshop and its implications. It was noticed that, by proposing an open, flexible activity to the user´s demands, it was possible to arouse artistic and management interests, which culminated in the engagement in a photography collective - something that was not due to an obligation to attend activities, but to an opening of the workshop to the interests of each participant, which worked best for increase "membership" and the frequency.(AU)

O vínculo terapêutico nas terapias cognitivas / The therapeutic relationship in cognitive therapy

O presente artigo, por meio de consulta à literatura específica sobre o tema, tratou de pesquisar a vinculação terapêutica na abordagem cognitiva. No desenvolvimento do trabalho, foram abordados os seguintes aspectos: como a aliança terapêutica foi concebida no contexto das terapias cognitivas; o papel do terapeuta na relação terapêutica; e como se constrói a vinculação terapêutica. A boa relação terapêutica é considerada por muitos autores como um preditor de sucesso do processo psicoterápico. No entanto, para se construir o vínculo deve-se considerar importantes aspectos, tais como: o cuidado precoce da relação, características específicas e individuais da terapia e do terapeuta. Ainda, constatou-se, na conclusão do artigo, a pouca produção na língua portuguesa e o impacto do vínculo na condição do terapeuta. No âmbito teórico da abordagem cognitiva: a diferença na importância e na ênfase da vinculação entre a terapia cognitiva de cunho racionalista e a perspectiva construtivista.(AU)

This review of the literature addresses the issue of therapeutic alliance within the cognitive approach. The following themes were addressed: how is the therapeutic alliance conceived in the context of cognitive therapies; the cognitive therapist's role in forming the therapeutic relationship; and how to build a therapeutic bond. A good therapeutic relationship is considered by many authors as a predictor of success in the psychotherapeutic process. However, to form a bond with the patient, some important aspects have to be considered, such as the early care with the relationship and specific characteristics of the therapy and the therapist. It was also concluded that the literature in the Portuguese language is sparse and that the bond has an impact on the therapist. It is also discussed that the rationalist and the constructivist approaches to cognitive therapy lay different importance and emphasis on the therapeutic bond.(AU)

¿Nuevos lenguajes en niños y adolescentes? Reflexiones a partir del Caso clínico de Luciano / New languages on children and teenagers? Reflections on Luciano’s clinic case

El siguiente trabajo pretende visualizar a partir del caso clínico de un adolescente, la importancia desde el punto de vista de la constitución psíquica, de todo aquello que ocurre en lo arcaico, centrando la mirada en el primer vínculo madre–niño. Visualizar de qué forma, algunas fallas que se pueden presentar en este vínculo, pueden generar lo que Winnicott denomina “deprivaciones” en la infancia, pudiendo producir una distorsión en el proceso de maduración psíquica del niño, impidiendo la posibilidad del pasaje a la independencia. Asimismo identificar las posibles consecuencias de estas “deprivaciones” infantiles -repetidas a lo largo de su desarrollo-, y la forma en la cual se podrían poner de manifiesto en la entrada a la adolescencia, con formas de vincularse a partir de conductas antisociales, con el intento de encontrar en el entorno aquello que percibió como algo que le fue quitado en sus primeros años de vida. Finalmente se culmina exponiendo la forma en la cual se realiza dicho proceso terapéutico, con encuadres modificables, pero manteniendo siempre una mirada de análisis psicoanalítica, centrando principalmente en el vínculo terapéutico como posibilidad de generar, a partir del mismo, formas de simbolización...

Starting with a teenager’s clinical case, the present work seeks to visualize the importance, from the point of view of the psychic constitution, of everything that happens in the archaic, focusing the attention on the first bond: the mother-child bond. It attempts to visualize how certain failures that may appear on this bond, may generate what Winnicott calls “deprivations” in childhood that may even produce a distortion in the process of the child’s mental development, preventing the possibility to become independent. Furthermore, it intends to identify the eventual consequences of these “deprivations” during childhood (repeated throughout the child’s development) and the way in which they may be brought to light when approaching adolescence, linking themselves through anti-social behaviors, trying to find in the environment all that he sensed that has been taken away from him during his first years of life. At the end, it finishes with the explanation of how this therapeutic process is carried-out, with adjustable contexts, but keeping always the focus on a psychoanalytic analysis, centered mainly on the therapeutic relationship as the possibility to generate different forms of symbolization, from this point of view...

Le présent travail essaie de visualiser, à partir du cas clinique d’un adolescent, l’importance dès le point de vue de la constitution psychique, de tout ce qui se passe dans l’archaïque, centrant le regard dans le premier lien: le lien mère-enfant. Il essaie de visualiser de quelle forme certains faillites qui peuvent se présenter dans ce lien, peuvent générer ce que Winnicott dénomme “déprivations” dans l’enfance, lesquelles peuvent produire une distorsion dans le processus de la maturation psychique de l’enfant, lui empêchant la possibilité du passage à l’indépendance. De même, ce travail prétend identifier les possibles conséquences de ces “déprivations” de l’enfance (répétés pendant tout son développement) et la façon dans laquelle elles pourront se manifester au début de l’adolescence, avec les différents formes de se lier à partir de conduites antisociales, en essayant de trouver dans l’environnement ce qu’il a noté comme quelque chose qu’on lui a été enlevé dans ses premières années de vie. Finalement, le travail fini en exposant la forme dans laquelle le dit procédé thérapeutique est réalisé, ayant des cadrages modifiables, mais gardant toujours un regard d’analyse psychanalytique, se centrant principalement dans la relation thérapeutique comme la possibilité de générer, à partir de cette relation, des formes de symbolisation...

Relação médico-paciente em oncologia: medos, angústias e habilidades comunicacionais de médicos na cidade de Fortaleza (CE) / Relation doctor-patient in oncology: fears, anguishes and communication abilities among doctors in Fortaleza, Ceará State, Brazil

A relação médico-paciente, em oncologia, adquire uma particular importância devido à gravidade da doença e ao estigma que muitas vezes acompanha a experiência do paciente. Esta pesquisa teve como objetivo analisar percepções e dificuldades que os médicos vivenciam frente ao paciente oncológico. Trata-se de um estudo qualitativo, baseado principalmente em entrevistas abertas e aprofundadas, realizado com vinte médicos que trabalham no Instituto do Câncer do Ceará. Baseando-se na convivência da Dra. Sheila com seus pacientes, Cássio e Elisa, personagens fictícios de uma crônica entremeada na discussão do artigo, ilustram-se os principais registros encontrados. Procurou-se abordar quatro temas centrais: a construção do vínculo, o desempenho de habilidades comunicacionais, a abordagem terapêutica e a interação com a família. O sofrimento e as implicações emocionais de pacientes e familiares, as diferenças comunicacionais existentes entre os médicos entrevistados, principalmente em referência à transmissão do diagnóstico e à terapêutica, tornam-se fatores importantes que influenciam a relação e o estabelecimento ou não do vínculo terapêutico. Diante disso, vê-se ser necessário estudos e debates acerca do tema, visando a uma incorporação desta temática na formação médica.

In oncology the doctor-patient relationship has a particular importance due to the gravity of the illness and to the stigma that is followed many times by the experience of the patient. This research was designed to analyze perceptions and difficulties that doctors face when they are dealing with oncology patient. It is a qualitative study, based mainly on open and in-depth interviews, involving 20 doctors from the Cancer Institute of Ceará. From the relationship of Dr. Sheila with her patients, Cássio and Elisa, fictitious personages of a chronicle larded in the quarrel of the article, the main facts are illustrated. We tried to approach four main subjects: the construction of the bond, the communication abilities performance, the therapeutic approach and the family interaction. The suffering and the emotional implications of patients and familiars, the differences in communication between the interviewed doctors, mainly in reference to the transmission of the diagnosis and the therapy, become important factors that influence the relation and the establishment or not of the therapeutic bond. Because of this, it seems to be necessary studies and debates concerning this subject, aiming at an incorporation of this thematic in the medical training.
